Registration

If you need to register your child at one of the schools in Bowie Unified School District, stop by the school’s office and complete a registration packet. For your convenience, you may also print out the registration packet and complete it prior to coming in to register. Some of the information we require includes:

  • Enrollment information
  • Home address
  • Home and work phone number
  • Medical records and health information
  • Emergency contact information

You must also provide the following documents to complete the enrollment process:

  • Student birth certificate
  • Proof of immunization
  • Transcript
  • Withdrawal slip
  • Proof of residency (utility bill or rental agreement)


Immunization Requirements

Arizona law requires students to receive certain immunizations in order to attend school. In the event of an epidemic, we will exclude children who have not been immunized . You may make arrangements with your family physician or with the county health department to immunize your child.
